Immigration Physicals

OccMedCIC has a seasoned United States Citizenship and Immigration Services (USCIS) Civil Surgeon on staff. Our office will complete an i693 form and provide it to the patient to submit to the USCIS.

Lab results, a copy of the completed form, and the sealed original forms are available within one week. If any vaccine information is missing, our office is able to provide immunization services to you as needed.

What to Expect

There is no need to fast before the immigration physical! Please enjoy normal food and drink before the appointment. The appointment will consist of:

  • An interview with the Civil Surgeon to discuss vaccinations and medical history
  • A brief physical exam
  • Blood tests and urine collection for lab testing. There is no need to fast prior to this exam. Please enjoy your normal food and drink before the appointment.

Included in the cost of the appointment is a blood test for tuberculosis for those 5 years and older.

Blood test for syphilis (RPR) and urine tests for gonorrhea and chlamydia will be added if required, based on current immigration physical exam guidelines.

What to Bring

  • i693 form: Fill out the first page and add your full name to the top of each page. Click here to be directed to the USCIS website.
  • Vaccination or Immunization record: Please click here for a complete list of vaccinations that are needed.
    Vaccines include for those > 18 years:
      • Td or Tdap within 10 years
      • MMR (documentation of two doses or titers for those born after 1956)
      • Varicella – two doses, titers or documentation of disease in a medical record (Varicella history)
      • Hepatitis B – documentation of three doses or a positive Hep B antibody titer

  • Government-issued photo identification: You may use a passport or a United States driver’s license.
  • Payment options: We do not accept cash or insurance for the exam. Please bring a credit or debit card to pay. We will offer an itemized receipt to facilitate reimbursement by insurance.

Our cost includes the required tests.
Immigration Physical Exam: $490 per exam

Any additional vaccinations or blood tests are an additional cost and can be reviewed with the provider.
